I've posted my custom character sheets on the Role-Playing Games section of this site. I've got one for Mutants and Masterminds and one for standard Unisystem (All Flesh Must Be Eaten, in particular). I'm also working on one for d20 Star Wars (the pdf available for that one is just awful, not to mention wrong).
